About Us
History
Lakeland Hospice & Home Care is a nonprofit, community-based healthcare organization that has been serving West Central Minnesota since 1983.
Our Hospice program began in 1983, and has evolved from a small volunteer-run organization to a Medicare-certified Hospice on the cutting edge of pain and symptom control, grief support, and holistic care at the end of life. We currently serve all of Otter Tail, Grant, Stevens, Traverse, and surrounding counties.
Our Home Care program began in 1985 as part of Lake Region Healthcare Corporation. Our Home Care staff provides rehabilitative services, including: skilled nursing visits; speech, occupational, and physical therapies; wound care; home health services, and IV therapy designed to return the patient to their best possible function.
In September 2006, Lake Region Healthcare Corporation, Lakeland Hospice, Inc., Broen Memorial Home and Pioneer Retirement Community partnered to form Lakeland Hospice & Home Care, Inc.
The four founding healthcare organizations will be able to do more good together, with increased staff, larger service area, and new organizational structure.
Location
The Lakeland Hospice office is located at 120 South Union Avenue in downtown Fergus Falls, just south of the intersection of Lincoln & Union.
We serve patients in all of west central Minnesota and in surrounding South Dakota and North Dakota areas.
