Understanding Home Care

What is Home Care?
The goal of Lakeland Hospice & Home Care is to assist individuals to remain within their own homes as participants in community life, by providing quality services as needed, safely and economically. Many times Home Care can help reduce medical costs. With appropriate medical care at home, it can mean less time in the hospital and recovery time after hospitalization can be shortened.

Who can benefit from Home Care?
Lakeland Hospice and Home Care provides care to people of all ages, including those who:

  • Were recently discharged from a hospital, nursing home, or rehab unit
  • Are elderly and coping with chronic disease or a new diagnosis
  • Are physically disabled
  • Are recovering from surgery
  • Are in need of IV therapy, such as antibiotics or hydration
  • Need instruction regarding a new medication or diagnosis.
Referrals are accepted from hospitals, continuing care facilities, physician’s offices, community agencies, patients, and family or friends.

 

What services are provided?
We are able to provide a range of services to meet each person’s home health care needs, including:

  • Skilled Nursing
  • Physical, Occupational, and Speech therapists
  • Medical Social Workers
  • Home Health Aides

 

Who is part of the Home Care team?
Home Care is provided by a closely knit team of physicians, nurses, home health aides, and social workers. Team meetings are held on a regular basis, to make sure the needs of every patient are being met efficiently and effectively.

The patient’s Primary Physician is in constant contact with our staff and oversees the medical care of their patients. Skilled Nurses assist with instructions about medication and infection control, change dressings, set up medications, assist with IV therapy, and wound care.

Therapists can provide physical, occupational, and speech therapy to improve or restore skills that were lost or diminished due to illness or accident.

Medical Social Workers can help patients and their families adjust to problems associated with illness or hospitalization, such as dealing with financial problems, arranging child health care, locating community resources, obtaining needed medical equipment, and counseling patients and families who are suffering with terminal illness or stress related to hospitalization or lifestyle adjustments.

Home Health Aides, under the supervision of a registered nurse, can provide intermittent personal care, assist with walking, and light meal preparation.

How do I begin receiving care?
The process begins with a phone call. We will arrange a meeting for further explanation of services and benefits. There is no charge or obligation on the part of the patient or the family for this meeting, and it can be scheduled at any time during the course of the illness in preparation for admission at a later date.
